Should I Switch to an Oral GLP-1?

PathPoint Health Blog

For years, GLP-1s were only available as injectable medications. However, a new GLP-1 pill (oral semaglutide branded as Wegovy® in a pill-form) has recently been approved by the FDA as the first daily oral medication for weight loss.

The recently published Phase 3 OASIS 4 trial evaluated oral semaglutide (25 mg once daily) for adults with obesity or who were overweight, showing a significantly greater mean weight reduction compared with placebo over 64 weeks.

With its proven efficacy, it raises the question:

Should I switch to an oral GLP-1—or take an oral GLP-1 at all?

Here’s what the science shows.

How do GLP-1 pills compare to GLP-1 injections?

Both oral and injectable GLP-1 therapies activate a glucagon-like peptide-1 receptor to:

  • Increase satiety
  • Slow gastric emptying
  • Improve blood sugar regulation

But how they’re delivered can affect absorption, convenience, and patient preference.

Who is an ideal candidate for an oral GLP-1?

An oral GLP-1 may be a good option if you:

  • Prefer a daily pill over injections
  • Are committed to consistent dosing and lifestyle changes
  • Are early in your weight-management journey

Because oral absorption can be more sensitive to food intake and timing, adherence to provider instructions is important for maximizing results. If you have gastrointestinal disorders, advanced metabolic disease, or have had mixed results with other therapies, an injectable option may remain preferable.

What does the evidence say about outcomes beyond weight loss?

Many people take GLP-1s for reasons other than solely losing weight. In published semaglutide studies, improvements in cardiometabolic parameters, like blood pressure, lipids, and inflammation, and quality of life have been observed alongside weight loss.

What are oral GLP-1 side effects?

Both oral and injectable GLP-1 therapies share similar potential GI side effects, with nausea, early fullness, stomach discomfort, constipation, and diarrhea being commonly reported. These side effects are generally mild to moderate and tend to lessen over time with dosing adjustments and medical management.

Where to get oral GLP-1s

While some companies offer online prescriptions with minimal clinical follow-up, getting a GLP-1 online doesn’t take into account a comprehensive medical assessment, monitoring of labs and vital signs, personalized dose adjustments and ongoing support for lifestyle changes.

Evidence shows weight management outcomes and cardiometabolic health improvements are best achieved when medication is integrated into a broader, clinician-supervised plan.

How much does an oral GLP-1 cost?

Pricing will vary depending on whether patients leverage their health insurance plan or are self-pay patients. Many commercially insured patients can anticipate paying approximately $25 per month with the initial Wegovy® savings offer, subject to a maximum savings of $100 per month.

For self-pay patients, the Wegovy® pill is available for $149 per month for the starting dose of 1.5 mg. Pricing varies depending on dosage.
