Your Care Team
You’ll be supported by an advanced practice provider (APP), who may refer you as needed to other members of our team including a registered dietitian (RD), and/or licensed clinical social worker (LCSW). This interdisciplinary team works together to guide your journey and provide the support you need to reach your metabolic health goals.
Costs & Insurance
Where we have in-network status, we will bill clinical visits through your insurance plan. Depending on your plan, you may have out-of-pocket costs like copays, deductibles or coinsurance. We’ll verify your benefits and review them with you before you begin.
Costs will vary depending on the care plan recommended by your provider during your initial consultation and would include the relevant copays, deductibles, coinsurance, or self-pay rate for each visit.
Yes! Many patients use HSA/FSA funds to cover our services.

You’ll meet with one of our specialized advanced practice providers (APPs) to review your health history, goals and any existing conditions. This helps us create a personalized care plan that works for you.
We have three locations in Middle Tennessee with more coming soon. PathPoint Health Nashville is located in the heart of Midtown, PathPoint Health Murfreesboro is located off Medical Center Parkway, and PathPoint Health Clarksville is located in the Saint Bethlehem area off Ted Crozier Sr. Blvd.
Program Expectations
If appropriate, your provider may discuss medication options, including FDA-approved treatments for weight and metabolic conditions. This could include GLP-1 medications like Wegovy® or Zepbound® (or Ozempic® or Mounjaro® if you have Type 2 diabetes), depending on your medical needs and insurance coverage.
