PathPoint Health expands Ascension partnership beyond Tennessee with new Jacksonville partnership
NASHVILLE, Tenn. (May 1, 2026) — PathPoint Health and Ascension St. Vincent’s today announced a new joint venture to bring integrated metabolic healthcare to Northeast Florida. The partnership marks PathPoint Health’s expansion outside of Tennessee and builds on its initial collaboration with Ascension Saint Thomas, where its footprint has grown to three clinic locations across Middle Tennessee. The first Florida location is expected to open in late 2026 at Ascension St. Vincent’s Southside in Jacksonville, with additional site developments across the region.
Since initially launching in July 2025, PathPoint Health has seen nearly 4,500 patient encounters, supporting patients in optimizing their overall metabolic health.

Historically, metabolic conditions, such as diabetes and obesity, were managed across multiple specialties. PathPoint Health’s care model was designed to integrate with patients’ existing providers, enabling more consistent management through longitudinal care.

PathPoint Health – Jacksonville Southside will offer medical management, nutrition and exercise planning, lifestyle counseling, advanced diagnostic assessments and ongoing care collaboration through a hybrid of in-person and virtual care visits.
