First two Middle Tennessee centers to open in June with additional expansion planned

Both organizations are committed to expanding access to comprehensive, evidence-based care for individuals struggling with metabolic health conditions, including obesity, diabetes, and related chronic diseases

NASHVILLE, Tenn. (April 8, 2025) – PathPoint Health, a provider of comprehensive metabolic care, and Ascension Saint Thomas, a leading health system in Tennessee, today announced a strategic joint venture to help individuals prevent and manage metabolic conditions, including diabetes and obesity.

The partnership’s first two centers will open in June 2025 in Nashville and Murfreesboro, offering medical care, exercise and nutritional planning, lifestyle counseling, advanced diagnostic assessments and care coordination—all within one specialty practice. Services will be provided through a hybrid model of in-person and virtual visits, covered by insurance, ensuring patients have access to convenient and affordable care.

“We founded PathPoint to address a critical need for evidence-based metabolic care that is integrated within the medical ecosystem,” said Grant Rutledge, founder and CEO, PathPoint Health. “Partnering with Ascension Saint Thomas, with its comprehensive footprint across Middle Tennessee, enables us to scale this care model quickly and bring it to more communities. Together, we’re enabling patients to access the right support at the right time to manage their health more effectively.”

Metabolic health challenges affect a significant portion of the U.S. population, with more than 42% of adults classified as obese, approximately 11% diagnosed with diabetes, and another one-third living with pre-diabetes. Despite these numbers, many still view chronic conditions as isolated health concerns rather than interconnected issues that share underlying risk factors for heart disease, stroke, and other complications.

PathPoint Health’s innovative approach emphasizes collaboration with a patient’s existing primary care and specialty providers. This coordination ensures seamless integration into the patient’s overall healthcare plan, leading to improved outcomes through consistent management and personalized, comprehensive treatment strategies.

“Obesity and complex chronic conditions require a comprehensive approach to care,” said Fahad Tahir, president and CEO of Ascension Saint Thomas. “With PathPoint Health, we’re building an integrated approach—coordinating primary and specialty care to create a comprehensive view of each patient’s metabolic health and address a critical need in our region.”

The first two centers will be located near Ascension Saint Thomas Hospital Midtown and Ascension Saint Thomas Rutherford and will open in June 2025. Additional sites are planned across Middle Tennessee over the next 12 to 36 months.
